If your matter/issue does not fit any of the explanations, select the "GENERAL AO CASE INTAKE" and fill it out.

When you are finished filling in the form, you will need to
- Print it ("Print Form" button appears at the end) and
- Bring that in to Student Legal Service's office,324 Illini Union, during regular office hours, 8:30 a.m. to 12:00 Noon, and 1:00 to 4:30 p.m.,
Monday through Friday,
- Bring your I-Card (UIUC Student ID) with you.
At that time you will be set up with an appointment to come back to meet with an attorney.
NOTE: Only UIUC students who are enrolled for the
current semester and have paid the S.O.R.F. Fee are eligible to use this service
Be sure to bring with you any relevant documents, such as- your traffic ticket(s),
- your Ordinance Violation ticket,
- your lease, or
- anything else that you consider relevant.

TRAFFIC AO CASE INTAKE
For Speeding tickets or other moving violations, DUI, Driving Under Suspended or Revoked license, jay-walking ticket, bicycle riding ticket,
but NOT parking tickets. SLS Attorneys are able to represent UIUC students only on tickets issued in Champaign County. Students
can consult attorneys on tickets issued in other counties in Illinois and other states, but attorneys cannot appear in court outside Champaign County.
ORDINANCE VIOLATION/CITY OFFENSE AO CASE INTAKE For tickets issued for underage drinking,
public urination, adult responsibility, Fake ID, keg violations, etc. In Champaign, the ticket is pink, in Urbana yellow (with second page pink).
HOUSING/LANDLORD/DAMAGE DEPOSIT AO CASE INTAKE For problems with your landlord,
your lease, the refund or non-refund of your damage/security deposit, sublessors, etc.
MISDEMEANOR AO CASE INTAKE For Misdemeanor charges.
We cannot represent you in Felony cases (a public defender will be assigned by the Court) or misdemeanor charges outside Champaign County.
POWER OF ATTORNEY AO CASE INTAKE
In order to obtain a Power of Attorney document, giving authorization to someone to handle your financial matters, especially while you are studying abroad.
HEALTH CARE/MEDICAL POWER OF ATTORNEY AO CASE INTAKE
In order to obtain a Health Care Power of Attorney, designating a person to make medical decisions on your behalf if you are unable to do so.

NAME CHANGE For consultation with an attorney about changing any part of your name.
GENERAL AO CASE INTAKE For everything else that does not fit in the other categories.
Your legal problem may be subject to a time limit.
You should not procrastinate in seeking
advice from an attorney.
- Only UIUC students who are enrolled for the current semester and have paid the S.O.R.F. Fee are eligible to use this service.
- For an appointment, you must come into the office personally. Be sure to bring in your I-card (student ID) to vertify your
identity and for us to check your fee status.
- You will need an appointment to speak to one of the attorneys; initial consultations cannot be done over the phone.
- Appointments are set up when you come in, for a different day; no walk-in consultations.
